Commitment to Privacy

As a registered Clinical Psychologist with the College of Psychologists and Behaviour Analysis of Ontario (CPBAO), I am committed to protecting your privacy and safeguarding your personal health information. I collect, use, and disclose personal health information in accordance with Ontario’s Personal Health Information Protection Act (PHIPA), the Personal Information Protection and Electronic Documents Act (PIPEDA), and the standards set by the CPBAO.

This page describes how I collect, use, disclose, and protect your information, as well as your rights regarding access to your health record.

Please note: Submitting a query or request through my website or via email does not create a formal therapeutic relationship. You are only considered a client once both you and I have consented to begin psychotherapy and a therapeutic relationship has been established. You are considered a member of the general public when submitting an initial request or query through the contact form on this website.

What Information I Collect and Why

I collect personal health information that is reasonably necessary primarily for the purposes of providing psychological services. This may include:

  • Contact information (name, phone, email, address)

  • Health history and relevant medical information

  • Psychological and relational history

  • Billing and payment information

  • Referring physician or health professional details (if applicable)

This information is used to:

  • Assess your needs and provide psychological services

  • Establish a baseline to track progress over time

  • Communicate with you about your care

  • Coordinate care with other health professionals (with your consent)

  • Manage administrative tasks (billing, scheduling, receipts)

  • Meet legal, regulatory, and professional obligations

Secondary Purposes

In addition to the primary purposes above, information may also be used for:

  • Invoicing, processing payments, and collecting unpaid fees

  • Confirming insurance coverage (e.g., session dates and fees, with your consent)

  • Professional accountability (e.g., College of Psychologists inspections, mandatory reporting of misconduct by other regulated professionals)

Safeguards and Protection of Information

Your personal health information is protected through physical, administrative, and technological safeguards:

  • Paper records (if any) are kept in locked storage and destroyed securely when no longer required.

  • Electronic records are stored on Jane’s secure encrypted servers in Canada.

  • Electronic devices are password-protected and secured.

  • Information transmitted electronically is encrypted or de-identified where possible.

Use of Jane Practice Management System

I use Jane, a Canadian practice management platform, to securely store and manage your personal health information. Jane is PHIPA and PIPEDA compliant, with industry-standard encryption and safeguards. Through Jane, I manage:

  • Scheduling and reminders

  • Clinical notes and health records

  • Billing and receipts

  • Secure communication

Your information in Jane is stored on secure Canadian servers and is accessible to me for the purpose of providing you with care.

Retention and Destruction

Client records are retained for 10 years after the date of last contact, in keeping with CPBAO standards.

When records are no longer required, they are destroyed securely (e.g., shredding paper records or permanently deleting electronic files).

Disclosure of Information

I will not share your personal health information without your consent, except in situations required or permitted by law. These include:

  • If you are at risk of seriously harming yourself or others

  • If a child is at risk of harm (as per the Child, Youth and Family Services Act)

  • If a resident of a long-term care or retirement home is at risk of harm (as per the Long-Term Care Homes Act and Retirement Homes Act)

  • If another regulated health professional has engaged in sexual abuse (as per the Regulated Health Professions Act)

  • If records are subpoenaed by a court of law

  • If required by the College of Psychologists & Behaviour Analysts of Ontario for quality assurance

  • If requested by law enforcement in connection with a missing person investigation

Whenever possible, I will discuss these limits of confidentiality with you.

Your Rights

You have the right to:

  • Access your health record (with limited exceptions)

  • Request corrections to factual information in your record

  • Add a statement of disagreement if corrections are not accepted

  • Withdraw or limit consent for the use/disclosure of your information (except where required by law)

Requests for access or corrections should be made in writing. I will respond within 30 days and explain if access is denied.

A reasonable fee may be charged to process requests for access or copies of records.

If you are unsatisfied with way your information has been dealt with and wish to make a formal complaint, you may do so in writing, and I will provide a prompt written response.
